Failed or damaged keys are the result of a loose rotor, not the cause.

Once a key is damaged, it may then prevent proper mating of the tapers.

That's why I always use a torque wrench when tightening a rotor nut. Well certainly since the time I had a rotor nut come loose many years ago and try to escape through the side cover. The rotor didn't slip though IIRC, the awful noise alerted me in time. I've never lapped one in first, perhaps I should start, though, (working) fingers crossed, I have no intention in taking one off again until the next belt change is needed.
